Why this matters
A heavy mirror that tore its anchor out of the wall is a load-path failure, and the right fix depends on where the studs are, how heavy the load really is, and whether the pull is shear or tension. A picture-hook anchor rated for a few pounds in shear can be ripped out the moment a load adds a tension or prying component, and most plastic expansion anchors fail catastrophically in drywall once a load works the hole oval. The decision is among three durable answers: land the fastener in a stud, use a properly rated hollow-wall toggle when no stud is reachable, or spread the load with a French cleat. Choosing wrong leaves a customer with a 60-pound mirror over a sink waiting to fall, which is a genuine injury and liability exposure, not just a cosmetic redo.
Symptom presentation
The hole tells the story. A clean round blowout the diameter of the anchor with crumbled gypsum dust means a plastic expansion or self-drilling anchor that simply could not hold tension. An oval, elongated hole means the load cycled and worked the fastener loose over time, often a too-light anchor for a swinging or bumped mirror. Cracked or dished paper around the hole means the load exceeded the gypsum's bearing strength in a concentrated spot. Note the mounting hardware: D-rings and wire concentrate load on two points and add a prying tension component, while a cleat distributes it along a line. Weigh or estimate the mirror: anything over roughly 20 pounds on a single point in drywall alone is living on borrowed time, and bathroom mirrors with heavy frames routinely exceed that.

Find the studs with a reliable stud finder and confirm by probing, then mark centers; in most homes studs sit at 16 inches on center, occasionally 24. Measure the mirror's hanging-hardware spacing and see whether at least one mount point can land on a stud. Sound the wall to estimate drywall thickness (1/2 inch is typical, 5/8 inch in some baths and ceilings), because toggle and anchor ratings depend on board thickness. Check for obstructions behind the wall in a bathroom: water lines, a medicine-cabinet rough opening, or a vent stack can rule out a stud location even when the finder reads solid. Note clearance behind the wall for a toggle's wings to swing, and confirm the wall is genuinely hollow drywall and not plaster-over-lath or a tiled surface, which change the fastener entirely.

Branch 1, a stud is reachable at one or both mount points: a lag screw or structural wood screw into the stud is the strongest, simplest answer. Use it whenever geometry allows. Branch 2, no stud lands under the mounts but the wall is hollow drywall: a true toggle is required. Snap-toggle or strap-toggle style anchors carry far more tension than plastic anchors because they bear against the back of the board over a wide footprint. Branch 3, the load is heavy, the spacing is awkward, or you want both points solid and only one stud is reachable: install a French cleat or a horizontal mounting cleat screwed into studs, then hang the mirror on the cleat. This converts a point load into a distributed line load and removes the prying component. Branch 4, the wall is tile, plaster, or masonry: drywall anchors do not apply, and the fix moves to tile/masonry fasteners.
Confirming diagnosis
Confirm a stud hit by drilling a small pilot and feeling continuous wood resistance and wood shavings, not a hollow break-through. Confirm toggle suitability by reading the anchor's published tension rating for the measured board thickness and comparing it, with a generous safety factor, to the mirror weight; a single 60-pound mirror should never sit at an anchor's listed maximum. Confirm cleat geometry by laying out the cleat across at least two studs and checking the mirror's top rail can capture the mating cleat. Confirm hollow clearance for toggles by inserting a bent wire and feeling for free space behind the board. For any tension-loaded mount, treat the manufacturer's static rating as a ceiling and design to a fraction of it, because real loads include bumps, leaning, and cleaning pressure.
A mirror over a sink, tub, or in a child's room is a personal-injury risk if it falls. Do not rely on plastic expansion anchors or picture hooks for any mirror over about 20 pounds. Use a stud-anchored screw, a rated toggle sized well above the load, or a French cleat into framing.
Remediation
When a stud is reachable, drive a structural wood screw or lag with a flat washer through the mount into solid framing, and shim the bracket plumb. When you must go hollow, install snap-toggle or strap-toggle anchors sized well above the mirror weight, repair the failed hole first with a setting-type joint compound or a backer patch so the new anchor bears on sound board, and avoid stacking a new anchor in the same blown hole. For the strongest distributed mount, screw a hardwood or aluminum French cleat into two or more studs, attach the mating cleat to the mirror back, and hang; this is the preferred method for large vanity and gym mirrors. Always patch and finish the failed original hole, verify the mirror is plumb and that the safety backing (cleat lip or J-channel) cannot lift off the wall when bumped, and add felt bumpers to keep the load tight to the wall.
