Sewage Category 3 Black-Water Decontamination per S500
Why this matters
Category 3 water under IICRC S500 is grossly contaminated water that may contain pathogenic, toxigenic, or other harmful agents. Sewage backups, river floodwater, and wind-driven rain that has contacted contaminated surfaces all qualify. The remediation differs from clean-water work at every step: PPE, containment, materials retained or removed, antimicrobial application, post-job verification. Treat sewage like Category 1 and you expose crew and occupants to biohazards (E. coli, hepatitis A, leptospira, parasites) and you leave a contaminated structure behind. This SOP is the field reference for a Category 3 response done to S500.
Confirming Category 3
S500 Category 3 includes: sewage from any source; toilet overflow including the trap; rising flood water from rivers or seas; wind-driven rain that has carried contamination; water from broken sanitary sewer lines, septic tank overflows, or contaminated mechanical systems.
Some grey-water (Category 2) sources escalate to Category 3 with time or temperature: dishwasher overflow, washing machine discharge, fish tanks, or any Category 2 water sitting more than 48 hours in warm conditions. Document the escalation in the file.
Initial safety stop
Before any technician enters the affected space: confirm utilities. Shut off electrical to the affected area at the panel; if you cannot positively confirm shutoff, lock out the affected breakers per OSHA 29 CFR 1910.147. Gas-fired equipment (water heaters, furnaces) in flooded basements may have damaged controls; shut off gas if there is any doubt and have a licensed gas technician inspect before restart.
Sewage in a confined space (basement with limited ventilation, crawlspace, sump pit) presents H2S and methane risks. Test the atmosphere with a 4-gas meter (O2, LEL, H2S, CO) before entry. Hydrogen sulfide above 10 ppm is OSHA TWA exposure limit; LEL above 10 percent is the action threshold; engage industrial-hygiene support if readings exceed thresholds. Confined-space entry per 29 CFR 1910.146 applies for any space meeting the definition.
PPE per OSHA and S500
For Category 3 work:
- Full-face respirator with P100 cartridges per 29 CFR 1910.134; supplied-air respirator for high-aerosol or confined-space conditions.
- Disposable Tyvek coverall (or equivalent ASTM F1671 barrier) covering legs, body, and arms; taped at wrists and ankles.
- Chemical-resistant gloves (nitrile minimum, doubled for tear protection); secondary cuffed glove over the coverall sleeve.
- Rubber boots or chemical-resistant shoe covers with rubber soles; taped to coverall legs.
- Eye protection if not in full-face respirator.
PPE is donned and doffed at a defined boundary; doffing follows a sequenced removal that does not contaminate clean clothing. Disposable PPE goes into biohazard bags at the doffing area; reusable items get decontaminated.
Containment
Set negative-pressure containment per S520 (mold) practice extended to Category 3 water: 6 mil poly walls, zipper-door access, HEPA-filtered AFD exhausting outside or to a contained capture, pressure delta target 5 Pa minimum. The containment isolates the affected zone from unaffected building space and prevents cross-contamination during demolition.
Air filtration uses HEPA filtration at 4 air changes per hour minimum, sized to the chamber volume. For aerosol-heavy work (cleaning, demolition of contaminated drywall), increase to 6 to 12 ACH.
Bulk water extraction
Extract bulk water before any other work. Truck-mounted extraction or portable extractors with biohazard-rated waste tanks; submersible pumps for standing water above 1 inch deep. Discharge to a sanitary sewer through an approved cleanout; never discharge contaminated water to storm sewer or to grade.
Verify the discharge connection is to sanitary, not storm; many municipalities prohibit any non-stormwater discharge to storm drains under Clean Water Act NPDES MS4 permits.
Material disposition: remove or clean
S500 default for porous materials affected by Category 3 water is removal:
- Carpet and pad: remove and dispose.
- Drywall: remove to at least 24 inches above the high-water line, or to the saturation line if higher.
- Wet insulation: remove and dispose.
- Particle board and OSB subfloor with direct sewage contact: typically remove.
- Vinyl flooring: typically remove; the substrate underneath is contaminated.
Semi-porous materials may be cleaned: dimensional framing, plywood subfloor not visibly absorbed, masonry. Method: detergent wash with hot water under pressure (within substrate tolerance), rinse, EPA-registered antimicrobial labeled for the surface and target organism, drying to baseline moisture content.
Non-porous (metal, glass, ceramic, finished hardware): clean and disinfect with an EPA-registered hospital-grade disinfectant per label.
Antimicrobial application
EPA-registered antimicrobials for Category 3 work must be labeled for the use site (sewage backup, flood restoration) and registered under FIFRA 40 CFR Part 152. Common products: quaternary ammonium compounds (Microban, Sporicidin), hydrogen peroxide-based (Concrobium, Benefect Botanical), iodophors. Apply per the label: dwell time (typically 10 minutes), surface coverage rate, ventilation requirements, PPE for application.
Antimicrobial application is not a substitute for physical removal and cleaning; apply only after the affected surfaces have been physically cleaned. Antimicrobials applied over contamination do not penetrate to the organisms underneath.
Verification and clearance
Post-remediation verification per S500: visual inspection (no visible contamination); moisture at baseline; surface sampling if specified (ATP swab for organic residue, microbial sampling for specific organisms if a health authority is involved). On large or high-risk jobs, retain an independent IEP for verification sampling.
Clearance documentation: written report with inspection results, moisture readings, sampling results if any, antimicrobial application logs, equipment used, waste disposal manifests.
Flood water that may have crossed agricultural land, oil or chemical facilities, or industrial sites should be sampled for specific chemical contamination (pesticides, petroleum, heavy metals) before treating as a routine sewage Category 3 job. EPA and state hazardous waste regulations under RCRA 40 CFR Part 260 can apply; in those cases the response transitions to a HAZWOPER (29 CFR 1910.120) operation requiring trained personnel.
Documentation
Daily psychrometric log; substrate moisture map; PPE doffing log; antimicrobial application record (product, EPA registration number, application rate, locations, dwell time); waste disposal manifests; photographs throughout; sampling results if any. The Category 3 file is heavier than a Category 1 file by design.
