Tankless Water Heater Flame Failure and Error Codes
Symptom
Customer reports "no hot water" or "hot water for a minute then it stops" on a tankless water heater. The unit may display an error code on the front panel or the remote (typical Rinnai 11, 12, 14, 16; Navien 003, 016, 030; Noritz 11, 12, 16; Takagi 111, 121). The fan or igniter may be heard cycling without sustained burner operation. In severe cases the unit locks out entirely after 3 to 5 ignition attempts and requires manual reset.
The dominant root-cause distribution for tankless flame-failure calls, drawn from manufacturer service data and field-tech reports:
- Flame-rod fouled (white scale or oxide layer) - roughly 35 percent
- Combustion-air or vent obstruction - roughly 20 percent
- Gas-supply issue (low pressure, undersized line) - roughly 15 percent
- Igniter or spark electrode failure - roughly 10 percent
- Heat exchanger scaled (limits operation, secondary flame issue) - roughly 8 percent
- Control board or sensor failure - roughly 7 percent
- Other (gas valve, blower, condensate) - 5 percent
The flame-rod is the leading single cause and the cheapest fix (often 5 minutes with a piece of 220-grit sandpaper). Service techs who skip flame-rod cleaning go straight to "needs a new unit" recommendations and lose deals.
How tankless flame detection works
Modern tankless units use flame rectification, not flame thermocouples. A small electrode (the flame rod) sits in the burner flame path. The control board applies an AC voltage between the flame rod and the burner ground. When the flame is present, the ionized gases conduct current asymmetrically (rectifying the AC into DC), and the board reads micro-amps of DC current.

The flame rod itself is a piece of high-temperature stainless or kanthal wire. Over time, an oxide layer or white aluminum-oxide-rich scale forms on the rod surface (from combustion products, especially in dirty-burning conditions). The oxide layer is electrically insulating: it does not affect the rod's ability to be in the flame, but it blocks the micro-amp signal from getting through.

Step 1: Read the error code
The manufacturer's service manual lists the specific fault by code. Common codes across brands:

A code 11 or 12 (or equivalent) is the flame-failure family and points to the rod, igniter, or gas supply. A code 16 points to flow restriction or scale. A vent code points to obstruction.
Step 2: Verify gas supply
With the unit calling but failing, measure gas pressure at the inlet test port (most units have a 1/8 inch NPT port). Required pressure varies by unit but typically 5 to 11 inches w.c. for natural gas at flowing condition (under load).
- Static pressure under 7 inches w.c. natural gas indicates a regulator or supply issue
- Pressure that reads correct static but drops below 4 inches w.c. when the unit fires indicates a gas-line sizing issue
- Pressure correct at the unit but no flame indicates downstream (gas valve, flame rod, igniter)
Gas line sizing: a 199,000 BTU/h tankless on a 30 ft run from the meter typically requires 3/4 inch gas line minimum, 1 inch in higher-demand or longer-run installs. A 1/2 inch gas line is undersized for any 199,000 BTU/h unit and is a frequent install error in retrofit applications.
Step 3: Inspect the vent
Tankless units use sealed combustion. Both the combustion-air intake and the exhaust must be clear and free of recirculation. Inspect:
- Termination: clear of snow, ice, debris, bird nests, spider webs (yes, spider webs do block 2-inch concentric vents)
- Length: total equivalent vent length (straight feet + elbow equivalents) must be within the unit's allowed maximum
- Slope: condensing units require condensate drain from the vent
- Recirculation: in some sidewall installations, intake and exhaust are placed too close and the unit ingests its own exhaust
Vent code 030 (Navien) or equivalent fires on detected blocked vent (typically a pressure-switch differential between expected and actual). Cleaning the termination usually clears the code; if not, walk the vent path looking for sags or filled condensate traps.
Step 4: Read the flame signal
If available, the service display or the manufacturer's connection software shows real-time flame signal in microamps. Strong signal at startup that drops over a few seconds indicates the rod is fouled (rectification works briefly until contamination disrupts the circuit). Zero signal from the start indicates the rod is fully insulated or the rod / electrode is broken.
If the unit does not display flame signal directly, an analog microamp meter in series with the flame rod ground wire reads the signal directly. Most service techs add this to their kit for tankless work.
Step 5: Pull and inspect the flame rod
