Confined Space Entry (OSHA 1910.146)

Why this matters

OSHA's Permit-Required Confined Spaces Standard (29 CFR 1910.146) applies to MANY routine service-trade environments - basement crawlspaces, attic chases, septic tanks, water heaters in tight closets, large HVAC plenums, commercial pipe vaults. Confined-space incidents kill 50 - 100 workers per year in the US, often from oxygen deficiency or hazardous atmosphere that the worker had no way to detect. OSHA citations for confined-space violations average per violation; willful or repeat violations far higher.

What counts as a confined space

OSHA's three-part definition:

  1. Large enough to enter + perform work - most trade-relevant spaces qualify
  2. Limited means of entry / exit - crawl access, manhole, hatch, tight stair
  3. Not designed for continuous occupancy - not meant to be lived or worked in

Common examples:

  • Basement crawl spaces with limited access (under 36" tall)
  • Attic chases + soffit cavities
  • Septic tanks (during pumping or service)
  • Water tanks
  • Underground utility vaults
  • HVAC equipment closets (specifically if door restricts emergency exit)
  • Boiler rooms with restricted access
  • Manholes + sewer vaults
  • Cisterns + water storage
  • Equipment pits

Permit-required vs non-permit

OSHA distinguishes two confined-space categories:

Non-permit confined space - confined but no atmospheric hazards. Examples: a clean dry attic crawl, accessible mechanical room. Service work allowed without permit but document the entry.

Permit-required confined space - confined AND one or more of:

  • Hazardous atmosphere potential (oxygen deficient, flammable, toxic)
  • Potential to engulf entrant (loose grain, liquid, sand)
  • Inwardly-converging walls (could trap entrant)
  • Any other recognized serious safety / health hazard

Permit-required spaces have stricter procedure.

Atmospheric hazards (the killer)

The #1 cause of confined-space fatality is atmosphere:

Oxygen deficiency

  • Normal air: 20.9% O2
  • Below 19.5%: OSHA defines as oxygen-deficient
  • Below 16%: impaired judgment, headache, dizziness
  • Below 12%: rapid unconsciousness without warning
  • Below 8%: unconsciousness in seconds; death in minutes

Common causes: rust + decay consume oxygen; nitrogen / argon / CO2 displaces oxygen; biological decomposition (septic, decaying vegetation).

Flammable / combustible

  • Below 10% LEL (lower explosive limit): safe
  • 10 - 100% LEL: flammable atmosphere; spark could ignite

Common causes: natural gas leak, sewer gas, fuel vapor.

Toxic

  • Carbon monoxide: lethal at 1,500 ppm; OSHA action threshold 35 ppm
  • Hydrogen sulfide: lethal at 100 ppm; rotten-egg smell but olfactory fatigue makes it deceptive
  • Methane: combustible + asphyxiant
  • Specific industrial gases per the work environment

Common in: septic + sewer (H2S + methane), HVAC after combustion equipment fault (CO), enclosed storage (varied).

Permit-required entry procedure

Pre-entry

  1. Identify all hazards for the space
  2. Atmospheric testing in this order:
  • Oxygen first (must be 19.5 - 23.5%)
  • Combustibles (must be <10% LEL)
  • Toxics (must be below permissible exposure limit)
  1. Pre-entry briefing of entrant + attendant + entry supervisor
  2. Permit completed documenting all hazards + isolation + monitoring + rescue plan
  3. Equipment inspection: monitor calibration current; ventilation; communication; rescue gear

During entry

  1. Attendant outside the space continuously watching the entrant
  2. Continuous atmospheric monitoring (instrument worn by entrant OR sampled regularly)
  3. Communication with entrant at all times (verbal, signal, radio)
  4. No bystanders in entry zone
  5. Forced ventilation if needed to maintain atmosphere

Exit

  1. Entrant signals exit
  2. Attendant confirms safe exit
  3. Permit closed
  4. Spaces secured against re-entry

Rescue

  • Most critical + most-violated requirement
  • Non-entry rescue: retrieval line + winch capable of extracting entrant without rescuer entering
  • Entry rescue (if non-entry impossible): trained rescue team with SCBA + harness + retrieval gear
  • The buddy who runs in to help an unconscious entrant becomes the second victim. This is the most common confined-space fatality pattern.

Equipment requirements

  • 4-gas monitor (oxygen, LEL, CO, H2S) - calibrated within last 30 days
  • Forced-air ventilator (if atmospheric hazards possible)
  • Communication (radio, signal line)
  • Retrieval system (harness + line + winch OR mechanical advantage)
  • Lighting (intrinsically-safe if combustible atmosphere risk)
  • SCBA / supplied air (entry rescue only; not for entrant under normal conditions)
  • PPE (chemical-resistant clothing, hearing, eye, hand)

Service-trade scenarios

HVAC tech entering attic to service AHU

  • Attic with restricted access via pull-down stairs counts as confined space
  • Hot ambient + insulation off-gas + spider / wasp encounter
  • Non-permit if atmosphere clear; document entry
  • DON'T enter alone if extreme heat or visible insulation off-gas

Plumber entering crawl space for slab leak repair

  • Crawl space under 3 ft tall counts as confined space
  • Possible atmosphere: sewer gas, methane, mold
  • 4-gas monitor recommended even in clean spaces
  • Communication with helper above-grade
  • Forced ventilation if work generates fumes (PVC primer, solvent welding, soldering)

Septic / sewer service

  • ALWAYS permit-required
  • Atmospheric testing before any entry
  • Forced ventilation
  • Attendant + retrieval
  • Specialized training required

Tank cleaning (water + chemical)

  • Permit-required
  • LOTO of fill / drain valves + electrical
  • Atmospheric testing + verification

Boiler room / mechanical closet

  • May be non-permit but document entry
  • Watch for carbon monoxide from failed combustion equipment
  • LOTO before service

Training

OSHA requires:

  • Entrants: hazard recognition, use of equipment, communication, evacuation
  • Attendants: monitor entrant, recognize emergencies, summon rescue, NEVER enter to rescue without proper gear
  • Entry supervisors: authorize entry, verify conditions, cancel permits

Refresher annually OR when conditions change.

NEVER enter a confined space to rescue a colleague without ATMOSPHERIC TESTING + appropriate PPE. The leading cause of multiple-fatality confined-space incidents is the well-meaning coworker who runs in to help the first victim. They become the second + third victim. The rule is hard: if your buddy collapses in the space, do NOT enter. Call 911. Begin non-entry rescue with retrieval line if rigged. Wait for trained rescuers with SCBA. Yes, the seconds matter - but you cannot help anyone if you're also unconscious. Train every employee on this principle until it's reflexive.
