A Safety Step Can't Be Verified Over the Phone: Decision Tree
Why this matters
Guiding a helper through a diagnosis is one thing. Guiding them through the safety step that has to be true before the next move - the circuit verified dead, the pressure relieved, the gas isolated, the energy locked out - is another entirely. When you cannot personally confirm that step is done and done right, you are about to send a person's hands toward stored energy on the strength of a word. A wrong number costs a trip. A wrong "it's dead" costs a life. This tree keeps the safety step from riding on faith.
Start here: unsafe until proven safe, and you stop, not guess
Lead with the action, not the diagnosis.
- If you cannot verify the safety step is done, treat the system as still live, still pressurized, still hot. Have the hand keep clear of the hazard and touch nothing that assumes it is safe. The default is danger; safety has to be proven, not assumed.
- Do not talk them into the hazardous step to keep moving. "It's probably off" is the sentence that precedes the injury. If it is not verified, it is not off.
Everything below is how to get real proof or, failing that, how to stop the right way.
What counts as a safety step you must verify
These are the steps where "I did it" is not enough, because being wrong is not recoverable.
- Verifying a circuit or conductor dead before contact.
- Isolating and confirming gas or fuel shut off.
- Relieving stored pressure - hydraulic, pneumatic, refrigerant, water - before opening a system.
- Locking and tagging an energy source so it cannot be re-energized.
- Securing a load, a height, or a moving part before someone reaches in.
If the next move puts hands, face, or body in the path of energy, the step that makes it safe is in this class.
Why a helper's "it's done" is not verification here
A green hand can believe a step is complete and be wrong in ways they cannot see.
- Verifying dead has its own procedure - test the tester on a known live source, test the target, test the tester again. A hand who "checked" may have skipped the live-dead-live proof and read a meter that was never working.
- The wrong point. They isolated a valve or a breaker, but not the one feeding this equipment. Backfeeds and mislabeled panels are exactly where this goes wrong.
- Partial energy. Pressure bled from one side, a capacitor still charged, a second supply still live. Absence at one point is not absence everywhere.
The narrow path to remote proof
Some safety steps can be proven live, and only live counts.
- A dead-check you hear in real time: they test the tester on a known source, test the target, test the tester again, reading each result to you as it happens. Not a report after - a live proof-test you walk through with them.
- A photo of the isolation: the lock and tag on the specific source, the valve handle across the pipe, the gauge at zero. An image of the state, not a description of it.
- Their own qualification: the live proof only counts if the step is within their training. A qualified hand proving it live is verification. Coaching an unqualified hand through a dead-check is not, no matter how clear the phone is.
If the live proof is clean and within their training, you may proceed. If any part of it is fuzzy, it is not proven.
If you cannot get live proof, stop and hand off the hazardous step
When the proof will not come clean, there is one answer.
- Hold the hazardous step for a qualified hand on site - drive out, send one, or reschedule. The diagnosis waits. The hazard does not get assumed away.
- Have the helper do the safe work in the meantime: gather nameplate data, secure the area, stage tools, so the hold is not wasted time.
- Never let an open ticket pressure you into "just have them check it." The whole point of this tree is that this is the one check you do not gamble.
