Heat Illness Prevention (OSHA NEP)

Why this matters

Heat illness is a growing OSHA enforcement priority + a serious workforce risk for outdoor + non-conditioned-indoor trade work. OSHA's National Emphasis Program (NEP) on heat illness, active since 2022, intensified inspection of construction + trades. California + Washington + Oregon + Minnesota + Maryland have state heat-illness standards; federal rule expected 2025 - 2026. Heat stroke is FATAL - 100+ worker deaths annually in the US. Prevention is the difference between a productive crew + a worker in the ER (or worse).

Heat illness spectrum

Heat illness escalates through stages - recognize early, intervene fast.

Heat rash (mild)

  • Cluster of small red bumps
  • Sweat trapped in skin
  • Move to cool dry area; allow skin to dry

Heat cramps (mild)

  • Muscle spasms (legs, abdomen, arms)
  • Result of electrolyte loss through sweat
  • Stop work; cool; hydrate with electrolyte beverage; rest

Heat exhaustion (serious - must intervene)

  • Heavy sweating
  • Weakness, dizziness, headache
  • Nausea, vomiting
  • Pale clammy skin
  • Pulse rapid, weak
  • Confusion possible

Treatment: stop work IMMEDIATELY; move to cool area; loosen clothing; cool with water + fans; give cool electrolyte beverage if conscious; monitor. If symptoms don't improve in 30 minutes OR worsen, ESCALATE - call 911.

Heat stroke (LIFE-THREATENING - call 911 immediately)

  • Body temperature 103°F+ (often 106°F+)
  • Skin HOT + DRY (sweating may have stopped) OR profusely sweating
  • Rapid strong pulse
  • Confusion, slurred speech, irritability, hallucination
  • Possible seizure
  • Loss of consciousness

This is a MEDICAL EMERGENCY. The worker can die within minutes. Treatment:

  1. CALL 911 IMMEDIATELY
  2. Move to coolest available area
  3. Cool the worker AGGRESSIVELY: ice water immersion if possible, ice packs to neck + armpits + groin, soak clothing with cool water, fan
  4. Loosen / remove clothing
  5. DO NOT give fluids if unconscious
  6. Stay with the worker until EMS arrives

Time matters. Brain damage starts at 105°F internal temp. Cool aggressively.

Risk factors (multiply when combined)

  • Ambient temperature 80°F+ (rises with humidity)
  • Humidity 50%+ (sweat doesn't evaporate; cooling fails)
  • Heat index above 91°F (OSHA NEP trigger)
  • Direct sun exposure
  • Heavy clothing / PPE (insulation traps heat)
  • Physical exertion
  • New worker (not yet acclimatized - first 7 - 14 days are highest risk)
  • Returning after vacation / illness
  • Medications (some BP medications, antihistamines, others reduce heat tolerance)
  • Underlying conditions (cardiovascular, obesity, alcohol use)
  • Personal factors (age, fitness, hydration history)

A heat-illness incident is rarely from one factor - usually a combination.

Prevention (the OSHA + state-recognized program)

Acclimatization

  • New workers + returning workers AT 50% workload for first day
  • Progressive increase to full workload over 7 - 14 days
  • Repeat after vacation 1+ week
  • Repeat after illness

Hydration

  • Cool water available at every job site
  • Workers drink 8 oz every 15 - 20 minutes during heat
  • DO NOT wait for thirst (thirst is delayed indicator)
  • Electrolyte beverages for heavy sweat days (Gatorade, Liquid IV, electrolyte tabs)
  • Avoid caffeine + alcohol before + during work

Rest cycles

  • High heat: 15 minutes rest per hour
  • Extreme heat: 20 - 30 minutes rest per hour
  • Rest in shade or air-conditioned vehicle
  • Adjust work hours to early morning OR evening when possible

Shade

  • Available at every job site for breaks
  • Vehicle interior + AC counts
  • Pop-up canopies on outdoor jobs

Buddy system

  • Workers in pairs at minimum during heat
  • Watch each other for symptoms
  • Report concern immediately

Training

  • All workers + supervisors trained on symptoms + response
  • Emergency procedures known
  • Heat-illness reporting protocol

Heat index thresholds (OSHA NEP)

OSHA NEP triggers inspector attention at heat index 80°F+, programmed inspections at heat index 91°F+.

Heat index combines temp + humidity:

  • 80 - 90°F + dry: caution
  • 90 - 105°F: extreme caution
  • 105 - 130°F: danger
  • Above 130°F: extreme danger

Direct sun adds approximately 15°F to perceived heat.

State-specific standards

California (Cal/OSHA T8 §3395)

  • Required for outdoor places of employment
  • Shade required at 80°F
  • Mandatory rest break + cool-down recovery period
  • Documented heat-illness prevention plan
  • Acclimatization period

Washington (WAC 296-307-097)

  • Outdoor employees at 89°F triggers requirements
  • Shade, hydration, monitoring required

Minnesota

  • Indoor employers at heat index 80°F+

Oregon, Colorado, Maryland

  • Similar requirements; check current state regulations

If you operate in these states, follow state standard (more stringent than federal). If you operate in non-regulated states, follow OSHA NEP guidance (functionally similar requirements).

Field implementation (small contractor)

  • Job-site cooler with ice + water at every job
  • Per-truck hydration policy posted
  • Heat-illness symptoms poster in every truck cab + jobsite trailer
  • Emergency response plan: 911 call procedure + nearest ER address
  • Buddy assignment for heat days
  • Schedule adjustment (early start; afternoon off when possible)
  • New worker acclimatization assignment

When to STOP work

OSHA recommends stop-work thresholds based on heat index + work intensity:

  • Heat index 90°F + heavy work: 15 min rest per 45 min work
  • Heat index 100°F + heavy work: 30 min rest per 30 min work
  • Heat index 105°F + heavy work: stop work or essential only
  • Heat index 110°F+: stop all non-essential work

These are guidelines. The worker who shows symptoms stops regardless of index.

NEVER assume a sweating worker is OK. Heat stroke can begin AFTER sweating stops - meaning the worker who suddenly looks dry on a hot day may be in the most dangerous phase. Watch for: confusion, slurred speech, irritability, stumbling. These are early heat stroke signs. Stop work, cool aggressively, call 911. The worker may insist they're "fine" - that itself is a symptom (heat-induced confusion). Don't argue. Cool + call.

Documentation + reporting

  • Heat-illness program written + distributed
  • Training records
  • Incident reports for any heat-illness symptom (mild OR severe)
  • Acclimatization schedule documented for new + returning workers
  • Annual program review
